Does your health plan require an assigned PCP (for example, an HMO)? If yes, verify that a Children’s Medical Group provider is your assigned PCP. This usually will populate on your Member ID Card.

Visits to our office are by appointment only. Everyone, including us, dislikes long waits in the office. Our schedule is designed to minimize such waits, but we do accept same-day appointments for sick visits. Please contact us to learn more.
Existing patients can schedule a well or sick visit online, minus the exceptions below*. Please note that you must type in your child’s name exactly as it appears in our records to be able to schedule an appointment — please do not use any nicknames or alternate names.
*Types of visits that cannot be scheduled online:
Any visits for an infant less than 2 months old
First evaluation for ADD or ADHD
Pre-operative clearance
Injuries, such as broken bones or sprains
Respiratory distress
Shot-only or lab-only visits

You can schedule well visits up to one year ahead of time.
At Children’s Medical Group, we try to get patients in and out of the office ASAP so they can get back to their busy lives — but without ever compromising the level of care we provide. The best times to schedule your visit are early in the morning or immediately after lunch (which ends at 1:30 p.m.). If we have a lot of sick visits or emergencies that day, you may wait longer than usual. To save time, you are also welcome to request a “first available” appointment.

Sick care
Yes, we see all sick children on the same day if possible. It’s always best to schedule an appointment ahead of time or call ahead for a same-day sick visit.
If your child is experiencing a life-threatening emergency, call 911.
Children’s Medical Group offers extended weeknight hours until 7:00 p.m. at select locations and Saturday hours (9:00 a.m. to 5:00 p.m.) at our Poughkeepsie location.
After-hours care is available by phone for all Children’s Medical Group patients. If your child has a medical problem that requires urgent treatment, you will be connected with a clinician who is trained to provide assistance by phone.
We recommend writing down the following information, so you can refer to it during your call:
Your child’s age
Your primary concerns
How long your child has experienced symptoms (and if they are getting worse)
Anything you have already done to treat your child’s condition
Your child’s temperature
Your child’s intake of fluids and output of urine and stool
Your child’s alertness (if you are concerned about lack of responsiveness, call 911 for emergency care)
We will also ask for your phone number so you can be called back if necessary.

Well visits
After your child turns 3, they begin getting annual checkups once a year, plus sick visits as needed. Scheduling annual checkups around your child’s birthday is a good way to remember!
During your child’s first year, well visits are scheduled 3-5 days after birth, 1 month, 2 months, 4 months, 6 months, 9 months, and 12 months.
During your child’s second year, well visits are scheduled at 15 months, 18 months, and 24 months.
During your child’s third year, well visits are scheduled at 30 months and 36 months.
For continuity of care, Children’s Medical Group encourages you to schedule all well visits with your child’s primary provider. However, we cannot guarantee that your primary provider will be available for same-day sick visits.
Telehealth
A telehealth visit is a video conference with a Children’s Medical Group provider that can often replace an in-office visit, based on certain qualifying conditions. Ask us or keep reading to learn more!

Children’s Medical Group provides virtual telehealth visits during regular operating hours — 9:00 a.m. to 5:00 p.m., Monday through Friday. (Please note, we are closed for lunch from 12:30 p.m. to 1:30 p.m.)
Yes, our providers can often treat minor illnesses, injuries, and other health concerns virtually. We will begin the visit by asking specific questions about your child’s medical history, then observe your child via video. In many cases, this process is enough to make a diagnosis and create a treatment plan. Your virtual provider also has access to your child’s medical records and can prescribe medication(s).
If your child needs an in-person visit for any reason, Children’s Medical Group will help you schedule one to ensure your child gets a complete and comprehensive evaluation.
It’s best to conduct the visit in a private, quiet, and well-lit area with a stable internet connection. Turn off the TV to minimize distractions, and put other children or pets in another room. For safety reasons, you should never conduct a telehealth visit from a moving vehicle.
We recommend preparing just like you would for an in-office visit, jotting down any questions or concerns ahead of time. We also recommend logging on a few minutes early in case there are any technical issues. Your child (the patient) must be present for the visit. Please also have your child’s current medication list and any other pertinent information nearby.
Yes, you will receive a text message and email confirming your child’s appointment details, including the date, time, and instructions for joining the virtual visit.
Telehealth visits are conducted via videoconferencing after calling your preferred Children’s Medical Group office to schedule your appointment. Existing patients are not required to pay any fees in advance. Your insurance company will be billed first and then you are responsible for any portion the insurance company does not cover. For self-pay patients, a telehealth visit costs $59.
Requests for forms, records, refills & referrals
If Children’s Medical Group has not seen your child for the condition you want a specialist referral for, you must schedule an appointment first. If your child has already been seen, you can visit the patient portal to make your request. We will contact you when the referral is ready, usually within 5-7 business days.
Contact our office and tell us what medication needs to be refilled, along with the phone number for your pharmacy. You can also request prescription refills via the patient portal.
Parents and requesters must use ScanStats’s Online Request Portal to submit a request for medical records. Patients and requesters usually receive the records 24-48 hours after submission. ScanStat provides bilingual customer support for anyone who needs help using the request portal and/or checking the status of the records. Email status@scanstat.com for assistance.

Patient portal
You can schedule and manage appointments, access your child’s medical records (including visit summaries, lab results, vaccines and immunizations, medications, and referrals), pay your bill, and more — all in one convenient place.
You will receive an email from Children’s Medical Group with a username and password that gives you access to the portal with any web browser or the healow™ app. (If you don’t see the email, please check your spam folder.) You can customize your password after your initial login.

Your practice code is included in the welcome e-mail for your family’s account. You’ll find it at the bottom of the e-mail.
Yes, all information stored on the patient portal is protected by industry-standard SSL encryption and the username and password of your choice.
Go to the patient portal, click on the “Trouble logging in?” link, and follow the prompts.
To see all patients under the same account, all patients must be listed under the same guarantor. To request this change, please email portal.east@pediatricassociates.com.
No, both the patient portal and the healow™ app are free for Children’s Medical Group patients, giving you easy access to your child’s medical records at no cost.
In the “Appointments” section, click “New Appointment.”
Select the visit reason and your Children’s Medical Group location.
You will see a provider list, plus the next available appointment.
Select the date and time you prefer and add your appointment details.
A 4-digit verification code will be sent to the phone number listed on your account.
Enter the code, then click “Verify and Book.”
You will see your appointment details in the “Dashboard” section of the portal.
In the “Medical Records” section, click “Lab Diagnostic Report’’ to view the results.
Click on the “Medical Records” section and then select “Immunization History.” You will have the option to print your child’s immunization records and other medical records in this section.
In the “Messages” section, click “Refill Requests.” You will see a list of your child’s current medications. Select the medication you need and click “Refill Request.” If no refills are available, we recommend scheduling an appointment in the “Appointments” section.
A proxy account allows a designated individual to access and manage another person's health information. Parents/legal guardians typically use this account to access their child(ren)'s records.
In adherence with the Cures Act, parents/legal guardians will receive login credentials that allow them to access multiple children’s information with one single login.
Note: Patients are eligible to have access to their own portal account after age 12, depending on state regulations.
