Request medical records

Parents and legal guardians:

To request your child’s medical records, you have two options:​

• Submit your signed authorization form via fax or bring it with you to our office. Other healthcare providers can also request your child’s medical records by completing an authorization form.​

• Use our ROI vendor, ScanStat, for full medical charts or additional documentation.​ Most requests are processed within 5–10 business days, though older records may take more time.

Third-party requestors:

• All third-party requestors must submit an Authorization for Release of Protected Health Information (available in English and Spanish). For patients under the age of 18, this form includes the signature of a parent or legal representative. In some cases, additional documentation may be required.

• Third-party requestors should use ScanStat to fulfill their medical record request.

Basic records
Basic records like visit notes, lab results, immunizations, and school/camp forms are available through the patient portal and may also be available for pickup at our office.

Entire chart

Requests for additional records or your child’s full medical chart must go through ScanStat. For fee estimates and other details, please contact ScanStat directly. Fees and processing time may vary depending on the request type and delivery method (digital vs. mailed).
