The Children’s Medical Group Receives NCQA’s Highest Accreditation As Patient-Centered Medical Home

Poughkeepsie, NY May 24, 2013:  The Children’s Medical Group (CMG) is proud to announce that it has been recognized as “Level 3 Patient Centered Medical Home” from the National Committee for Quality Assurance (NCQA). Level 3 designation is the highest achievable recognition for a medical group.

As Joseph Heavey, CEO explains, “NCQA’s Patient Centered Medical Home program recognizes physician practices that prioritize the physician/patient relationship, coordinate care for patients across multiple settings, and engage in a team approach to improve patient care. NCQA’s Level 3 PCMH accreditation reflects our efforts to ensure The Children’s Medical Group’s patients receive exceptional, accessible treatment that is well-coordinated among everyone involved in their care. This accreditation demonstrates that we are successfully making The Children’s Medical Group a true ‘medical home’ for our patients.”

The medical home concept encourages all of a patient’s medical providers to coordinate care and work together to ensure that appropriate care is given. All medical information including records, test results and referrals to other care providers, including specialists, are housed with the patient’s primary care physician and made available to other involved specialists. Benefits of this approach include less duplicate testing, which saves time and money.

The Children’s Medical Group was surveyed by the NCQA on nine standards that focus on patient care, outcomes and quality. These standards include:  

  • Care management
  • Patient self-management support
  • Electronic prescribing
  • Test tracking
  • Referral tracking
  • Performance reporting and improvement
  • Advanced electronic communications
