New Requirements for Student Health Examinations

Dear Parents/Guardians,

As of July 1, 2018, New York State law requires a health examination for all students entering the school district for the first time and when entering Pre-K or K, 1st, 3rd, 5th, 7th, 9th , and 11th grade.

The examination must be completed by a New York State licensed physician, physician assistant or nurse practitioner.

A new approved NYSED Student Health Examination Form for Schools will also be available beginning on 7/1/2018.

Many medical provider offices may not yet be aware of the change. To assist with the transition, schools may accept any health examination form provided during the 2018-2019 school year.

If you have already had a physical examination completed for the 2018-19 school year on our traditional CMG School/Camp/Daycare form, the school may accept the physical on that form.

The new approved NYSED Student Health Examination Form will be the only form accepted starting in the 2019-2020 school year.

The Children’s Medical Group will be collaborating with our EHR vendor and all local school districts to help in the transition into the new mandated NYSED Student Health Examination form for the 2019-2020 school year.
