On-Line Prescription Refill Request
We offer two convenient options when refilling your child's prescription - by phone and on-line.
Please be aware that regular medical supervision is required to refill some prescriptions and you may be asked to make an appointment for your child to be examined and the treatment plan discussed. For controlled substances, your child must have been seen in our office for a medical evaluation within the past three months - NO EXCEPTIONS.

To make it easier on both you and our staff, we have a dedicated prescription refill line. When you call our office, please follow the prompts to reach the refill line. On the line, please leave your name, the patient's name, your phone number, the name of the medications, the strength of the medication, the dosage instructions (e.g. 1 tsp. 3 times a day), the pharmacy name and phone number, and whether you would like the medicine called in, or a written prescription picked up (specify which office) or mailed to your home. Please keep in mind that a controlled substance prescription can not be called in. The line is checked twice daily Monday - Friday during the hours of 8:30 am - 5:00 pm, and the prescription will be processed in about 48 hours. Prescription requests received on Friday, weekend days and holidays, will be handled Monday or next business day. Prescription refills are NOT done on the weekend and holidays.
Please be sure to include all of this information. Failure to do so will make it impossible to fulfill your refill request.
On-Line Prescription Refill Request
By agreeing to the terms of our Prescription Refill Request Form, you will enter a secure area of our site. The Children's Medical Group will be held harmless should your electronic message not be transmitted due to technical problems related to this site or hosting server. All personal identifying information is encrypted and your message will not be internally or externally forwarded to other third parties.
We will respond to your request within 48 hours, Monday - Friday, during business hours, 8:30 am - 5:00 pm. Prescription refills are NOT done on weekends and holidays. For those prescription requests received on Friday and weekend days, your prescription will be handled Monday or next business day. If you do not hear from us within that timeframe or should have any questions regarding your prescription refill or this on-line form, please contact us by phone at (845) 452-1700.
If you do not accept the terms of this site disclaimer, you will not be able to process your prescription request on-line.
